COVID-19 WAIVER AND LIMITATION OF LIABILITY
I, (parent’s full name) desire to use/have my child participate in the Brookfield Academy Junior Knights (Junior Knights) Football club program for the 2020 season, and acknowledge and agree to the following:
- Novel coronavirus (COVID-19) infections have been reported throughout the United States and have caused state and federal authorities to take substantial action in coordination with the Center for Disease Control (CDC) to limit the spread of this virus. To comply with these orders, the Brookfield Academy Junior Knights Football Club has taken measures to comply with all orders from state and federal authorities and the recommendations of the CDC.
- The Brookfield Academy Junior Knights Football Club is committed to doing ensuring the safety of all players, coaches and others that participate in the Junior Knights’ football season. As such, Brookfield Academy Junior Knights Football Club has implanted detailed policies (see Appendix A) to ensure that it provides an atmosphere that is as clean and safe as possible.
- However, the Brookfield Academy Junior Knights Football Club cannot prevent you, your children, or other individuals from becoming exposed to, contracting, or spreading COVID-19 while participating in the Junior Knights’ football season. It is not possible to prevent against the presence of the disease. Therefore, if you or your minor child(ren) choose to participate in the Junior Knights’ football season, you may be exposing yourself/your child(ren) to and/or increasing the risk of contracting or spreading COVID-19.
- By signing this waiver, you hereby affirm that no one participating in the Junior Knights’ football season including yourself and any child(ren), have within the past 14 days:
- returned from any highly impacted areas subject to a CDC Level 3 Travel Health Notice,
- been exposed to any person returning from any highly impacted areas subject to a CDC Level 3 Travel Health Notice,
- been exposed to any person who has a suspected or confirmed case of COVID-19, or
- experienced symptoms of COVID-19 including, without limitation, fever, cough, or shortness of breath.
- By signing this waiver, you hereby consent to any reasonable action taken by the Brookfield Academy Junior Knights Football Club to ensure the health and safety of you, your child(ren), and other people participating in the Junior Knights’ football season, including, without limitation, to have your/your child(ren)’s temperature taken or to wear a protective mask when appropriate.
- Despite being aware of the risks involved in participating in the Junior Knights’ football season in light of COVID-19, you voluntarily desire and agree to participate in the Junior Knights’ football season. You agree that should you or your child(ren) sustain contract COVID-19 or any related symptoms, the Brookfield Academy Junior Knights Football Club is not liable for any damages arising therefrom.
- In considering the privilege to participate in the Junior Knights’ football season, you, on behalf of yourself and all minor children participating in the Junior Knights’ football season, agree that you, your spouse, domestic partner, heirs, assignees, guardians, or legal representatives WAIVE ANY RIGHTS TO make any claim against the Brookfield Academy Junior Knights Football Club or any of its officers, coaches, and players in interest for any compensatory, consequential, incidental, indirect or special damages or account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by participate in the Junior Knights’ football season during a period of COVID-19.
- You further agree to INDEMNIFY, DEFEND, AND HOLD HARMLESS the Brookfield Academy Junior Knights Football Club, and any of its officers, coaches and players in interest harmless from ALL CLAIMS, ACTIONS, OR DEMANDS, whether or not such matters are filed in a court with proper jurisdiction or through arbitration, that may result from your or your child(ren)’s participation in the Junior Knights’ football season and its related services, including but not limited to claims related to COVID-19.
- You acknowledge that the Brookfield Academy Junior Knights Football Club has the right to refuse participation in the Junior Knights’ football season to any person who in the sole discretion of the Brookfield Academy Junior Knights Football Club officers or coaches evidences any symptoms of COVID-19.

Appendix A: Brookfield Academy Junior Knights Football Club COVID Guidelines and Protocol
Junior Knights’ COVID Guidelines:
- Anyone (players, coaches, parents, etc.) with any COVID like symptoms; cough, sneezing, sniffles, etc. (CDC Symptoms Link) should not come to practice or to games.
- When not practicing or participating in a game, players and coaches are expected to maintain social distances (6’ or greater).
- Water bottles will not be provided by the Junior Knights’ coaches during practice or games. Players are responsible for bringing their own water to games and practices.
- Players must take extra care to NOT share their water bottles or equipment.
- Players must separate their equipment (6” or greater) from other’s equipment during games and practices.
- Coaches and parents must wear a mask when within 6 feet of another person. Players, not wearing a mask, are permitted to be within 6 feet of another player or coach only with their helmet on.
- Players and coaches will have their temperature taken before each practice and game. If a player or coach has a temperature over 99.6°, they will not be allowed to participate that day.
- Hand sanitizer will be available for players and coaches at all practices and games. Please use frequently.
- Used footballs, and equipment will be disinfected (wiped down) periodically and after practice and games.
- NOTE: As we monitor the COVID situation, we may need to make changes to these guidelines.
Junior Knights’ COVID Protocol:
- Any player or coach who has had close proximity with another who has a confirmed case of COVID must notify a member of the Junior Knights coaching staff immediately and either self-quarantine for 10 calendar days or provide a negative test result to a member of the Junior Knights coaching staff before returning.
- Any player or coach testing positive for COVID must notify a member of the Junior Knights coaching staff immediately.
- If any player tests positive for COVID, that player’s entire team will have their season suspended (practices and games) for 10 calendar days.
- If any coach tests positive for COVID, and that coach has been confirmed to have had close proximity with any player, the entire team will have their season suspended (practices and games) for 10 calendar days.
- Please be aware that we will not disclose the names of anyone testing positive.
- A communication will be sent out to all team parents when any COVID related activity has been reported.
- All incidents will be handled individually.
